Jeszcze zanim dziecko zostanie przyjęte np. do pracowni ceramicznej, miasto zbierze o nim mnóstwo danych (https://pozaszkolne.edu.pl/zwpek_mdk/index.php?idpoz=1), w tym sporo wrażliwych np.:

  1. Czy pochodzi z wielodzietnej rodziny (oznacza to rodzinę wychowującą troje i więcej dzieci);
  2. Czy jest dzieckiem niepełnosprawnym;
  3. Czy jego rodzice są niepełnosprawni;
  4. Czy ma niepełnosprawne rodzeństwo;
  5. Jak wyglądają relacje między jego rodzicami, cytat z formularza: „(...)oznacza to wychowywanie dziecka przez pannę, kawalera, wdowę, wdowca, osobę pozostającą w separacji orzeczonej prawomocnym wyrokiem sądu, osobę rozwiedzioną, chyba że osoba taka wychowuje wspólnie co najmniej jedno dziecko z jego rodzicem”.
  6. Czy jest wychowywany przez biologicznych rodziców, cytat z formularza: „objęcie kandydata pieczą zastępczą”.

Takie dane trzeba podać, aby zapisać się do publicznej placówki w Warszawie. Trzeba też przygotować stosowne dokumenty, które potwierdzą informacje zawarte w deklaracji. Lista domów kultury wraz z indywidualnymi linkami do formularzy https://pozaszkolne.edu.pl/
